The attached forms have been added for your convenience.

A transcript request form is needed for all transcripts.  If a student is not 18 years old, a parent/guardian signature is required.  Please allow at least 24 hours for processing.  Current students are permitted up to six transcripts per request.  Transcript request froms are also available in the counseling office.

The Parent Brag Sheet is a supplemental form that I will use when writing letters of recommendations in addition to the student's resume.
